SRS Facilitator Sponsorship Form

Every potential certified SRS Facilitator must have the support and backing of their organization before stepping into this training and role. This is accomplished with a supervisor of the potential Facilitator completing the sponsorship form below. This form is not a pass/fail test but rather helps us confirm your organization’s awareness and support and the suitability of the applicant to fill the role of a certified SRS Facilitator.

To facilitate a Bootcamp requires certain skills and requirements. Do you agree that the applicant is able to: (check all that apply)
- Facilitate a group of 20+ (Facilitating is different from lecturing)
- Competently speak in public
- Has the skills to plan a 2-day event with 20 participants, location, and catering • Or there is someone available to assist the event planning
- Has the availability to devote 80 hours to prepare for their initial Bootcamp event
- Has the availability to attend the required week-long Support Raising Leaders Conference every 18 months
- Has the availability to meet recertification requirements of attending a public Bootcamp every 18 months
